We maintain and uphold HIPAA compliance protocols across every collection, test, and report we handle.

Information We Collect

Depending on the service requested, we may collect: identifying information (name, date of birth, government ID), contact information (address, phone, email), employer or sponsoring organization details, chain-of-custody and specimen data, test results, medical review information, and payment information. For background checks, we collect information necessary to fulfill the requested search in accordance with applicable law.

How We Use Information

We use collected information to perform requested screening services; report results to authorized parties (such as the donor, employer, Medical Review Officer, or designated laboratory); maintain chain-of-custody documentation; comply with DOT, Non-DOT, and other regulatory obligations; respond to lawful requests; and operate, secure, and improve our services.

HIPAA Compliance

Protected Health Information (PHI) is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state law. We apply the minimum-necessary standard, train staff on PHI safeguards, restrict access to authorized personnel only, and execute Business Associate Agreements where required. PHI is never sold.

Disclosure of Information

We disclose information only to: the individual to whom it relates; the employer, agency, or organization that ordered the test as authorized; certified laboratories and Medical Review Officers involved in result verification; and government or legal authorities when required by law, subpoena, or court order. We do not sell or rent personal information to third parties for marketing purposes.

Data Security

We maintain physical, administrative, and technical safeguards designed to protect information against unauthorized access, loss, misuse, or alteration. This includes secured facilities, chain-of-custody protocols for every specimen, restricted-access recordkeeping, encrypted electronic transmission of results where applicable, and ongoing staff training. No method of transmission or storage is 100% secure, but we work to maintain industry-aligned protections.

Retention & Disposal

Records are retained for the period required by applicable federal, state, and contractual requirements (including DOT recordkeeping rules where applicable). When records reach the end of their retention period, they are securely destroyed in a manner designed to prevent reconstruction or recovery.

Your Rights

Subject to applicable law, you may have the right to request access to your records, request an amendment to inaccurate information, request an accounting of certain disclosures, and request restrictions on certain uses. To exercise these rights, contact our office using the information below.

Children's Privacy

Our services are directed to adults and to organizations testing adult donors. We collect information about minors only when a parent, legal guardian, or authorized agency requests testing services on the minor's behalf, and we handle that information in accordance with applicable law.

Third-Party Labs & Partners

Certain tests require analysis or verification by certified third-party laboratories or Medical Review Officers. Those entities are independently responsible for their own privacy and security practices, and we share only the information necessary to perform the requested service.
